*Important Note: We request that you supply only the name of your bank or credit union, not your bank account name or account number. We will send you a periodic debit authority specific to your bank. Rather than giving us the authority to take money from your account, you fill out the form and take it to your bank to authorise your bank to pay the quarterly payments for you. That way you stay in control. We feel this is safer for you as we cannot take any more money from your acount if you should cancel the authority for any reason.
